When Should You Refer a Wound Patient to a Specialist?

As a primary care provider, you see patients with wounds regularly. Most heal with standard care. But some don't — and knowing when to refer can make the difference between healing and amputation.

Signs a Wound Needs Specialist Attention

Consider a wound care referral when any of the following are present:

• The wound has been present for 4 or more weeks without measurable improvement

• The wound is enlarging or deepening despite appropriate treatment

• There are signs of infection not responding to oral antibiotics

• The wound involves exposed tendon, fascia, bone, or capsule

• The patient has diabetes and any lower extremity wound — regardless of duration

• Standard dressing changes are becoming unmanageable in your practice setting

• The patient is immunocompromised, malnourished, or has significant vascular disease

Why Early Referral Matters

Chronic wounds — those present for 30 days or more without meaningful progress — rarely resolve without a change in approach. The longer a wound remains open, the higher the risk of biofilm formation, osteomyelitis, and ultimately amputation. In diabetic patients, lower extremity amputation carries a five-year mortality rate that rivals many cancers.

Early referral to a certified wound care specialist allows for advanced debridement, bioengineered skin substitutes, negative pressure wound therapy, and coordinated care with vascular surgery — interventions that are difficult to access through primary care alone.
